Insilico Medicine's rentosertib was built by AI to treat idiopathic pulmonary fibrosis, a disease where lung tissue scars and stiffens until breathing fails, usually within three to five years of diagnosis. The company generated the drug candidate using its Pharma.AI platform, which proposed both the biological target and the molecule's structure, then ran it through a placebo-controlled trial. The New York Times reported on September 7, 2026 that alongside improved lung function, patients on the drug showed reduced markers of biological age, the epigenetic and molecular signs that a cell has aged past what a birthdate alone would predict.
The mechanism worth sitting with is that nobody set out to build an anti-aging drug. Idiopathic pulmonary fibrosis and biological aging appear to share upstream cellular machinery, senescent cells that stop dividing but refuse to die, and a molecule aimed at one showed effects on the other. That is a hypothesis generator, not a therapy: this is one trial, in lung patients, measuring age markers as a secondary observation rather than the thing it was powered to prove. The next gate is a trial designed from the start to test rentosertib, or a similar compound, against biological age as the primary endpoint, in people who do not have pulmonary fibrosis.
